Targeted intervention CAMHS Community Team Consultation. Assessment Targeted intervention

Secondary school offer 27 secondary schools involved. The Zone – Progeny are co-ordinating project. Mental Health Lead Half a day CAMHS clinician. Young Devon counselling service. Kooth online counselling.

CAMHS Outreach Team COT work 7 days a week from 8am – 8pm. Offer assessment in the acute setting within 24 hours of referral being made. Offer follow up post assessment in the community. See all urgent referrals to CAMHS within 7 days Manage CYP in crisis or with high risk in the community. Aim to prevent admission to Tier 4 unit. Offer targeted support to reduce crisis and manage risk behaviours

Plymouth MDT Complex pathway CYP with complex MH needs who are not clinically ready for a specific therapy but need MH monitoring or support before receiving therapy. Outcome based interventions for up to 12 weeks Intervention team CBT Art/creative therapy Family Therapy Psychotherapy Eating Disorder pathway Early Intervention Psychosis (EIP) pathway

Neuro-developmental Team Multi –Disciplinary Team (MDT) outpatient assessment for 5-18 years with suspected neuro-developmental disorder and co-morbid MH presentation. Intervention work and family support for CYP with a diagnosis of ASC/ADHD with co-morbid mental health issue Children’s day Programme 5-12 years 2 full days over 4 weeks assessment days for complex neuro/mental health presentation Neuro Crisis Team 2 week response for CYP with a diagnosis who are going into crisis (not risk or MH crisis)

Specialist Teams Infant Mental Health Team 0-5 years Consultation Assessment Intervention Children in Care Team 5-19 years Consultation Assessment Intervention Foster carer support Co-working with CSC colleagues. YOT liaison post Severe Learning Disabilities Team 5-18 years Consultation Assessment Intervention Family support Liaison and direct work into the special schools

Plym Bridge House 12 bedded regional Tier 4 inpatient unit commissioned by NHSE. CYP aged 12- 18 years with acute mental health presentation that cannot be managed in the community. CYP PoS unit for all aged CYP detained under (S)136 of MHA
