Cardiff Local Health Board Bwrdd Iechyd Lleol Caerdydd Designed for Life Level 3 Specialised and Critical Care Centre Level 2 Local Acute Services Level 1 Services provided at home, in the community or in supported housing. These will include Primary Care Resource Centres, social care support services and intermediate and rehabilitation services Level 4 Highly specialised services including social care

Cardiff Local Health Board Bwrdd Iechyd Lleol Caerdydd Core Principles service led and not estates led – a resource centre may be virtual or a physical building. focus on development of health care that can safely be provided in the community in support of a network of local primary care teams developed to reflect local and natural communities and respond to local needs as appropriate reflect agreed locality planning and delivery areas i.e. will serve a network of GP practices linking to a hub of other services maximise opportunities to achieve improved integration with social care, voluntary sector and other services ensure that existing capacity and new capacity is optimised to meet the needs of the local population, reducing inappropriate duplication of resources where possible.
