Purpose Since contrast sensitivity is deteriorated significantly in proportion to the area of anterior capsule opening, surgeons should attempt to prevent severe anterior capsule contraction. The purpose of this study was to examine the effect of two or three Nd:YAG laser relaxing incisions made in the anterior capsular rim on prevention of anterior capsule contraction after cataract surgery.

Methods Patients 100 patients scheduled for bilateral cataract surgery randomized to one of two groups a) Two or three relaxing incisions in the left eye at 3 days after surgery, and no incision in the right eye b) Performed oppositely Outcome measures 1) Area of the anterior capsule opening (mm2) 2) Percentage reduction of the opening area (%) 3) Visual acuity and refractive status 4) IOL decentration (mm) and tilt (°) 5) Degree of posterior capsule opacification (PCO) At 3 days, and 1, 3, and 6 months after capsulotomy

Conclusions 1) Three Nd:YAG laser relaxing incisions made in the anterior capsule decrease the anterior capsule contraction, whereas two relaxing incisions do not. 2) Degree of IOL decentration and tilt, or PCO does not worsen after Nd:YAG laser relaxing incisions.
