Instructions for Retention Storyboard: A Focus on Strategies to Improve Retention Rates Each Storyboard should consist of 5 slides: 1 cover slide that identifies your agency, 4 additional slides that explain your quality improvement activities focused on retention Please select ONE of the 2 categories below and then follow the appropriate guidance for slide construction. Retention rates are under 90%: Describe your plans to improve your rates using the plan-do-study-act model (guidance on next slide) Retention rates are at or above 90%: Share the activities you used to reach and maintain this rate (guidance on third slide) Please feel free to personalize your slides! Add color, pictures, graphs, designs….anything that helps share your effort and shine your stars!

(Slide 1 is your cover slide that identifies your agency) If Your Current Rate is Under 90%, Please Share Your Current OR Future Strategy For Improving Retention Rates (Slide 1 is your cover slide that identifies your agency) Slide 2: PLAN Slide 3: Do State your objective, and briefly describe your improvement plan, including who will do or has done what, how, and when! Describe how you carried out or will carry out your plan. If the plan has been carried out, describe any documented observations. Slide 4: Study Slide 5: Act Show how you analyzed or will analyze your results. What information did you collect or plan to collect, and how can it be used? Show your retention rate data here. Describe any needed changes or adjustments to your test. If it was successful, describe how to implement it on a larger scale. Or if not yet implemented, describe any anticipated outcomes or challenges.

If You are Already at or Above 90%, Please Share the Strategies You Have Used to Achieve and Maintain High Retention Rates (Slide 1 is your cover slide that identifies your agency) Slide 3: Data management (for example: provide details on how you need to collect, store, manage, analyze, and report data. How did you share your data and who did you share it with?) Slide 2: Clinical policies, procedures or protocols (for example: describe any changes in policy and procedures, adjustments in standard operating procedures, changes in clinic routine, etc.) Slide 4: Staff education, leadership involvement &/or involvement of external partners (provide details about who was involved and how they were engaged) Slide 5: Other (provide any other information relevant to your successful improvement efforts. This might include a systems change, new partnerships, working with an EMR, etc.)
