The Community Health Worker serves as a valuable resource to the other network members (primary care and hospital). Patient discharged from hospital with specific instructions. CHW assures compliance. Chronic non-compliant clinic patient. CHW counsels and “educate” patient on behalf of primary care provider. CHW provides Health Ed. at health house/post location.

The Community Health Worker visit patients in their homes at the request of primary care providers or hospitals who are in the integrated network. These visits often prevent the need for more costly interventions such as emergency room treatments or hospital admissions/readmissions.

UnitedHealthcare Enrollee Without Community Health Worker Hospital Scenario 2 UnitedHealthcare Enrollee Without Community Health Worker Hospital Local Clinic Health House/Post Scenario 2 UnitedHealthcare member targeted for Health House Network intervention. The Community Health Worker helps non-emergency patient select the most appropriate site for the problem that is presented. In many cases the condition can be resolved at the health house. Inappropriate Hospital ER Utilization, Frequent Hospital Re-admissions, Increased Cost $$$$$! More Appropriate use of System Improved Quality, Reduced Cost $$!
