Welcome to Trinitas eScheduling! A new service from Trinitas Hospital that allows you to send requests for tests and procedures to our Central Scheduling Department Benefits: Make requests when convenient for you Bypass busy signals Batch your requests to enter all at once Print Confirmation pages of requests made Patient Pads* available for patients to make requests Live Chat available for immediate assistance** (*see Slide 12; ** in most cases)

eScheduling works for medical groups and individual physician practices. Patients can also request their own tests and procedures using Trinitas eScheduling!

OR, Sleep Center and Wound Center requests are not permitted within this application Requests are NOT confirmed tests until we check the schedule and get back to you (and/or your patient)

MEDICAL GROUPS If you have more than one doctor in your practice and you want to schedule for all of them, you can do so with a single user name and password. NOTE: If you are doing scheduling for a single physician, you can skip this slide.

NOTE!! Click here to get to the actual form! Batch your requests with this form; enter them into our system when it’s most convenient for you. Request more Patient Pads from Central Scheduling NOTE!! The Batch Request Form is not intended to fax to Central Scheduling. Rather, it is to be used during the course of a day to enter information that you can sit down at a later time and enter into our web site, one request at a time. DO NOT FAX THIS FORM TO CENTRAL SCHEDULING!!

Patient-Centric Information We only have 10 fields of information that are Required (*), so entering your patient’s information can be done quickly. Once you’re done, hit the Submit button and you will get a Thank You page you can print and append to your patient’s file, pending confirmation of the day/time from Central Scheduling.

Print This Page! We urge you to print this page and keep it as a record of your request. We generate a unique ID for each request, so if you need to call Central Scheduling you can refer to this number for internet-related requests only. This number does not correlate to phone or faxed requests. And, if you need to schedule tests for other patients, or additional tests for this patient, use this link to get to a new form to make another request.

What Happens Next? Your request is sent to us via email, and it also drops into a database where our schedulers can pick up requests from two different sources. This also enables us to make sure all requests are received. We check our internal scheduling system for the day and time your patient wants, and we will work diligently to get all the information we need to move this request forward and turn the request into an actual test. Trinitas wants you to refer as many patients as possible with the confidence of knowing we will do whatever it takes to help you help your patients.

We are hoping you will display them prominently at your Registration Desk or other areas where you display info. Patient Pads -------------- If you prefer that your patients make their own scheduling arrangements, we’ll need you to fill in the shaded box info for the Test/CPT and the Diagnosis/ICD9 information we’ll need to fulfill the patient’s request.

Batch Request Forms ------------- Download and print quantities of this form on legal sized paper to help you group your requests. Then, later in the day, or in the evening, you can enter in the info via our web site when it’s most convenient for you
