Informed Consent Before taking part in this study it is important that you understand: This study aims to evaluate the benefits of lessons on sexually transmitted infection (STI) in your school You don’t have to participate, its completely up to you If you do, you’ll be asked to complete three short questionnaires over a few months Some of the questions ask you your opinions on having sex without condoms and whether you have or would do this When completing the questionnaires you can choose to leave out questions if you don’t want to answer them
Informed Consent cont. We will make sure that you are seated so that others can’t see your answers You won’t be asked to give your name but we will collect details like your postcode, date of birth etc Even if you decide to participate, you can stop at any time, without giving a reason Quizzes will be provided along with questionnaires for those who do not wish to participate At each time point you will be asked to seal questionnaires / quizzes (whether completed or not) in an envelope and to post this in a secure box
Informed Consent cont. When received by researchers your answers will be entered into a password protected computer database The questionnaires will be stored in locked filing cabinets; they will be destroyed once we’ve finished with them Your parent(s) or guardian(s) have been told about the study and are happy for you to take part Please take time to read the information sheet before deciding if you want to take part; if you want to take part, you’ll be asked to sign a consent form Any questions?
