Research What information did you learn throughout your research? Why is this topic important? Why should others take an interest? Is there any statistical information that you can provide to support your idea? Again, why should people care about this topic?

Mentor Information Provide a brief introduction to your mentor Why did you choose this person? What is this person’s specialty in this area? Why are they an expert? What is this person’s experience in this area? Contact Information

Mentor Hours What did you do during the time that you spent with your mentor? Give an overall explanation of what you did while working with your mentor What of interest did you learn during this time? Identify items that were new to you and helpful in completing your final product

Final Product Describe the process in which your final product was created? Provide sufficient details for the panel to understand this process How will this product benefit yourself and/or others?
