LTI no 27: Finger crushed 13/June/2000 Operator tried to remove the bolt from this cross-over flange. He lifted the very heavy assembly (55 kg) but had to drop it down again.

LTI no 27: Finger crushed 13/June/2000 The tip of his finger got trapped and crushed between the studbolt and the flange. He was transferred to Nizwa hospital for corrective surgery. He will be off work for at least 2 weeks. Auch !!!!

LTI no 27: Finger crushed 13/June/2000 What caused this LTI: 1) Operator tried to lift/move a load that was too heavy for him. He should have used a crane. 2) The operator failed to see the hazard of his finger being trapped in between the long bolt and the flange. Discuss in your team: - Do you ever lift heavy objects manually ? Why ? - How would you have removed the bolt ? - How can you “see” the hazard/danger of a task/job ?
