Working Together Across Cheshire Is a programme of work to: Support the development of Integrated Care Partnerships (coalitions of health and care providers) in Cheshire West and Cheshire East to deliver more joined- up care closer to home. Strengthen integration and collaboration across the four Cheshire CCGs in move towards a potential single CCG. Support better outcomes and experiences for patients. Why do we need to change? Care quality – we’re not currently meeting some NHS Constitution standards. Unprecedented levels of demand for services. Limitations in our ability to quickly deliver improvements. Examples of fragmented, inconsistent care remain. Funding for health and care services is tight. Demand for services rising faster than budgets. Significant system-wide challenges. Cheshire CCGs face annual challenge to balance the books. Integrated Care Partnerships Partnerships of multiple organisations delivering health and care services in the same geography – true system approach with collective accountability for budgets and outcomes. ICPs are designed to join up hospital and GP services, health and social care, mental and physical health, and the NHS with local communities. Development of ‘care communities’ which build on networks of GP Practices working together with community care teams to deliver care closer to home. Greater emphasis on supporting prevention, promoting self-care and self-management of long-term conditions. Services built on assets of local communities with more involvement of patients and users in service design and delivery. How we plan to get there Timeline to April 2020 – ambition agreed by all four Cheshire CCGs. Ongoing engagement with CCG Member Practices, the pubic and our stakeholders. Strengthen existing collaborative commissioning arrangements – overseen by Joint Commissioning Committee. CCG functions and services support the development of the Integrated Care Partnerships Working together across Cheshire: NHS Eastern Cheshire Clinical Commissioning Group (CCG), NHS South Cheshire CCG, NHS Vale Royal CCG and NHS West Cheshire CCG

Have Your Say For a number of months all Cheshire CCGs have been discussing the proposals with clinicians, partner organisations and patient groups and forums. To enable members of the community to have their say, a short online survey will be accessible via each CCG’s website between 28 May and 23 June 2019. Paper copies of the survey can be requested from workingtogetheracrosscheshire@nhs.net We are inviting people to share their views and to stay involved by working with us to shape how we involve patients and the public in our work into the future.
