NB: combinations increase risk No Consider other risk factors and refer to appropriate service as required Yes Discuss with Falls Champion before referral Contact NHS Norfolk Medicines Management Team via pauline.shead@norfolk.nhs.uk or carol.dack@norfolk.nhs.uk Yes Is the person resident in a care home? No Who dispenses their prescriptions? Prescriptions dispensed by local pharmacy Prescriptions dispensed by Dispensing GP Complete referral form for GP appointment Give referral form and information to patient. Patient should visit pharmacy for Medicines Use Review (MUR)
