Presentation transcript:

What do we need to do if an injury/incident or accident happens in our workplace Franchisee needs to communicate the injury/incident or accident to their BC as soon as practical. By the end of the shift the franchisee is to complete the incident report form and provide this to their BC (all circumstances). Franchisee is to determine if the injury/incident or accident is required to be notified to work safe. If it is the franchisee must complete the online ‘notifiable event form’ and advise the BC of the relevant number. As part of the incident investigation ‘if required’ the franchisee is to complete the hazard identification form and provide this to the BC

What tools are available to ensure we all follow the same process? Quick link access on Moogle will help to navigate you to the below forms and flow charts. Incident reporting – process –flow chart. This is a step by step guide of the process that needs to be followed when an injury/incident or accident happens Franchisee – accident incident – near miss report form. This is to be completed at the end of the shift and sent to your BC. Also file a copy for yourself in your health and safety folder. Hazard identification form – This form may be required to be filled in as part of the incident identification process. http://www.worksafe.govt.nz/worksafe/notifications-forms/notifiable-events - link to the work safe NZ website.
