Organ Donation Dr. Ihab Harbieh

n Ahmad from Jordan, he is the only son of a family with 8 daughters. He had congenital stricture in the ureter, so he had renal failure. Nephrologist said that he will be luck if he lives. But the child is still alive. After 14 years he needed kidney transplant. His sister has matched tissue with him, but she will marry next month, and may it will be pregnant. So that was a dilemma. So you have to count to 10 before say anything to the patient. All patient with kidney transplant will receive immunosupressants forever (like Mycophenolate mofetil (Cellcept) 500mg & Prograf . Prograf tacrolimus is an agent can cause seizures. New kidney can be effective for 15 years.

we are as doctors we should know What is donation? When should we do it? Donation & ethics Right of patient & relatives 8500 out of 30 million only will donate with their organs after death

Xenograft: a tissue graft or organ transplant from a donor of a different species from the recipient. (from animals) (not used due to the risk of infection & mutation) Artificial organs are not effective ( heart ill not work more than 1 hour), they are ethically justified. Chimeras: half human

In the Netherlands, this process is related to autonomy and it can be done by simple registration on a specific website. The role of relatives: e.g. 18 years old guy, give his consent for donation before his death, his family refuses that. So here, we as a doctors should talk to family and try to change their mind (in ethical point of view). In Spain, Australia and other countries If you did not say that I do not want to donate, they will take your organs after death. Reciprocity: the practice of exchanging things with others for mutual benefit, especially privileges granted by one country or organization to another. So If you are registered as a donor, so you are eligible receive an organ. But if someone have HIV HCV so he can not be donor (non ethical) There is a consent for donation should be applied by family in Saudi arabia.

If somebody is 70 years old he can donate if he had healthy kidney Kidney can be saved within 24h with special solution
