Tell them your child will be entering the 7th grade to ensure compliance for next year’s requirements. The immunization records can be submitted to the clinic via fax, e-mail, or in person beginning now! *Please send all immunization records to the MIS/HPMS nurses and Athletic physicals to the coaches!
