Items needed before starting report: Salary Information Contract Begin & End Dates Base Salary Health & Retirement Benefits Other Duties ie. Bus Driver, Class Sponsor, etc. Enrollments by grade level (Unduplicated) Daily Class Schedule Begin & End times of each class period Begin & End dates of each class The grade level you teach for this class period The number of males and females in the class period

Screen 1: Verify that everything on this screen is correct: continue without saving.

Screen 2: Fill in Total Years of Experience that apply to you.

Screen 3: Fill in the information needed: Save & Continue.

Screen 4: You have to have at least 1 number listed Screen 4: You have to have at least 1 number listed. Select it as your primary contact: Save & Continue.
