Long Beach Refinery Client INCIDENT REVIEW Long Beach Refinery Client In 2012, this client site has experienced 16 injuries to hands and arms. A few statistics about these injuries include: 4 of the 5 recordable injuries have been to hands and arms 5 of the 16 were to client employees, 11 to contractors The injuries have happened to operators, craftsmen and office workers 5 of the injuries happened in the first 6 months of the year. In the 3½ months since, they have experienced 11 injuries to hands and arms 11 of the injuries could have been prevented by using gloves or by using the proper type of glove The last bullet point may be the most important statistic of all. When we permit work and perform work, we need to assure we are using gloves when called for and using the proper type of glove.

Pictured to the right is a leather glove of the type provided by the client. Leather is good for normal work such as with abrasion hazards like material handling, climbing ladders, using most tools and opening/closing valves. Leather provides good protection to abrasion. However, leather does not provide much cut protection. To the left are Majestic cut level 3 gloves with Dyneema (a lightweight, extremely tough fiber that is 15 times stronger than steel). Cut resistance level is based on standard tests and ranges from level 0 to level 5. These gloves can feature materials like Dyneema, Kevlar, Hexarmor SuperFrabric and even metal mesh. Generally, the higher the cut resistance, the bulkier and more cumbersome the glove becomes. So, simply specifying a higher cut resistance is not always the right answer to assure the safest completion of the job.

Long Beach Refinery Client INCIDENT REVIEW Long Beach Refinery Client Remember: Use gloves for every job where there is a hazard to your hands. When permitting work, include the specifics of glove type in mutual understanding discussions. Remember, your hands have no fear. They’ll go wherever your brain tells them to go. Keep your head and hands in the same place.

SAFE ZONE The Safe Zone is a gathering point for all employees in the event of an emergency evacuation. In the Safe Zone, a headcount will be conducted to account for all employees. All employees are to remain in the safe zone until the “all clear” has been issued from authorities and/or management.

SAFE ZONE The Safe Zone for PCS will be the grassy area directly to the north of the PCS driveway on the north side of Lakeshore Dr.
