siren Social Interventions Research & Evaluation Network SIREN’s mission is to catalyze and disseminate high quality research that advances efforts to address social needs in health care settings. Catalyzing high quality research First let me tell you a little bit about SIREN. SIREN stands for Social Interventions Research & Evaluation Network. Our mission is …. SIREN is essentially a national research acceleration and translation initiative where we both catalyze and disseminate high quality research to advance our understanding of how to address social and economic risk factors in the context of health care delivery. We do this mostly by doing three things: catalyzing high quality research by funding research and serving as a hub for researchers, disseminating research findings and tool through an online evidence library and monthly newsletter and providing evaluation, research and analytics consultations to folks who are looking for guidance about how to evaluate their interventions. Disseminating research findings and tools Providing evaluation, research & analytics consultation sirenetwork.ucsf.edu | siren@ucsf.edu | @SIREN_UCSF siren
SIREN Referral Platforms Project Dec 2017 – Nov 2018 Identified platforms Health care clients Resource directory and referrals Talked to: Vendors Users Others researching this space Funded by three TX funders: EHF, St Davids and MHM
So the first thing I have to share is that this area really is a moving target. Companies are changing their products regularly. It seems that new companies are popping up every few months. Some organizations are merging. It’s very much an evolving field so that what is true today, may not be true in a few months.
11 platforms in our review Eleven
~3 Types Referral platforms for health care providers (Healthify, Reach, NowPow, One Degree) Resource directories and/or collaboration networks but not specifically for health care sector (Aunt Bertha, Charity Tracker, Unite Us) Care and referrals management for health care expanding into social services (CrossTX, LivWell, Pieces Iris, TAVHealth and more)
Basic functionalities
How they differ Audience Focus on directory, referrals, and/or network REACH: directory and outgoing referrals only NowPow, Healthify: directory and bi-directional referrals Unite Us: network approach LivWell: care management Public facing or not Cost Geographic reach In general are all quite
Opportunities More efficient identification of resources and referrals Much better data about referrals and outcomes More research and evidence Improved patient care and outcomes
Challenges Closed loop is hard to implement Health care driven implementation may not focus on needs of community partners Potential for multiple different platforms being used in the same geographic area Unclear if these will be accessible to settings that most need it.
