How do we track & improve System Performance Measures in Pima County? Use Coordinated Entry, including diversion strategies, to assess and to identify housing and support to meet the needs of individuals and families experiencing homelessness Update By Name List to identify each person and household who becomes homeless, tracking monthly inflow & outflow into the homeless services system, including those who return to homelessness Conduct and track Outreach efforts throughout CoC, including HMIS data entry Utilize best practice strategies to reduce the time it takes for someone to access housing Expand Coordinated Entry efforts to include hospitals, school districts, etc.

Improving System Performance Measures (Cont.) Expand TPCH participation, including mainstream resources, to connect individuals and families with necessary supports to remain in permanent housing Encourage utilization of PHA HPP vouchers through TPCH move-on strategy Maintain high data quality to maximize the ability to accurately report system changes
