Do you have a fear of falling? Do you want to be more active? Then this is the programme for you Includes: Home Environment Assessment Physiotherapy Assessment Educational talks from the multi disciplinary team Setting individual targets Home exercise programme 9 weeks of exercise sessions and target reviews Advice and education Text message service (FLO) to motivate with targets and exercise Feedback ‘ My confidence is a lot better, marvellous programme, I’m fitter both physically and mentally’ ‘ I feel my walking has improved, can I come back?’ ‘I’ve been able to walk further than I have for 2 years’ Contact your local physiotherapy/ occupational therapy department or falls nurse
