Maricopa County and the City of Phoenix -Tina Luke and Mark Carr Youth Program Models Maricopa County and the City of Phoenix -Tina Luke and Mark Carr

Model and Service Locations Maricopa County – Elements Contract County Staff handle Outreach, Recruitment, Eligibility, Enrollment, Service Referrals, and Case Management. Contracted Agencies Deliver Service Elements Including Follow up: ACYR, CPLC, ResCare and DK Advocates dba ANB Services Service Locations: WVCC, EVCC, Tempe, Mesa, Surprise, and Wickenburg. Itinerant Services also provided at the Peoria Community Center and at the Avondale Rio Salado Adult Basic Education program. City of Phoenix – Program Contract Contracted Agencies handle Outreach, Recruitment, Eligibility, Enrollment, Service Provision, Case Management and Follow up. Service Locations: ACYR, CPLC, JFCS, and YMCA. Referrals also come from the following One-Stop Locations: West Phoenix, South Phoenix and North Phoenix.
