THE COMMUNITY PARTNERS PROGRAMME (CPP) As we get older we may choose to get support in the home or in the community. Later we may want to live in an aged care facility. Organisations that provide support to the elderly are called “aged care support service providers”.

WHAT DOES THE CPP WORKER DO? The CPP worker is the link between the aged care services and the Hungarian community We tell aged care services about the Hungarian community And we tell the Hungarian community about aged care services Aged Care Service Providers CPP Nelke and Julia Hungarian community

WHY? SO THAT ELDERLY HUNGARIAN PEOPLE HAVE GREATER ACCESS TO CULTURALLY APPROPRIATE AGED CARE SUPPORT SERVICES HOW: The CPP worker gives information to the Hungarian community on aged care services so you can make good decisions AND The CPP worker teaches Aged care services about the Hungarian community and their needs so they understand you better
