Deeside hazard reporting system THE PROCESS. All employees are issued with personal booklets and briefed on site objectives - classification of hazard, near miss & challenge. Books are coded to each employee but names are not communicated to others, to enable anonymous reporting. Hazard / near miss identification encouraged through regular engagement. All Hazards generated are presented at the daily production meetings. Identification information (optional) Location and classification of issue All information recorded and entered into database which is communicated to all employees on a regular basis Description of the issue raised What you did immediately after noticing the issue and how we can remedy Scoring system 1 – Minor risk but will likely not happen again 2 – Minor risk and medium potential of re-occurring 3 – Minor risk and will certainly occur again 4 – Major risk and low potential of re-occurring 5 – Major risk and medium potential of re-occurring 6 – Major risk and will certainly occur again Record and monitor issue Record and investigate issue Immediate investigation required, work may be stopped in the area
