Summary GP Patient Safety Incidents October 2016 - December 2016 Medication Patient Safety Incidents: 27 cases 52.9 % of all reported Patient safety Incidents Reported across Eleven (11) GP Practices 57.9% of all NHCCG practices

Others: No INR monitoring Delay in Diabetic Meds to Care Home Meds not removed from e-repeat prescription Delay in pharmacy dispensing from e-prescription. Meds (Vitamin D) taken daily not weekly Duplicate child vaccination Off licensing prescribing Self medication with depo provera
