Benign Breast Papilloma without Atypia: Outcomes of Surgical Excision versus US-guided Directional Vacuum-assisted Removal or US Follow-up For benign papilloma without atypia, the upgrade rate to malignancy was 1.9% after surgery, 2.1% after US-guided directional vacuum-assisted removal (DVAR), and 0% with US follow-up without intervention (P = .80) The recurrence rate after US-guided DVAR was 3.6%; all recurrences were confirmed as benign at surgery. In the group with US follow-up without intervention, 59 of the 61 lesions (97%) remained stable in size with no growth during a mean follow-up of 43.3 months; two lesions increased in size and were confirmed as benign at surgery.
