TCPi participation/engagement TCPi journey and successes Preparedness to participate in APM APM participation/arrangements The Value-Based Compensation Initiative (VBCI) is Arkansas Blue Cross and Blue Shield's new provider payment model intended to improve the quality, affordability and sustainability of healthcare by moving from volume-based to value-based compensation. BCBS VBCI provides a broader participation allowance than its predecessor Episodes of Care which was phased out December 2018 *****Discuss NEA’s participation in VBCI***** Comprehensive Primary Care Plus (CPC+) is an advanced primary care medical home model which boasts a unique public-private partnership consisting of 56 aligned payers currently supporting 2881 participating primary care practices across 18 regions spanning multiple states.

CPC+ Journey & Successes 5 key comprehensive functions 2 Tracks 3 Payment Elements Participation Requirements NEAs CPC+ Journey Comprehensive Primary Care Plus (CPC+) CPC+ focuses on 5 key functions as related to primary care which includes -Access and continuity -Care management -Comprehensiveness and coordination -Catient and caregiver engagement -Planned care and population health. CPC+ is divided by two tracks. Track 1 – Lower PBPM Care Management fees and incentive metric based payments, 100% FFS Office Visits, HIT Partners not required Track 2 – Higher PBPM Care Management fees and incentive metric based payments, 100% FFS Non-E/M office visits, hybrid up front & lower FFS for E/M office visits, HIT Partners are required This model includes 3 payment elements: Care management fee, Performance management fee and payment under the Medicare physician fee schedule.
