Locality Improvement Plan 6 Key Outcomes Improved access to GP’s and other services. Supported older people. Reduced social isolation. Supported carers. Mental health is supported. Promotion of healthy living.
Key Achievements 6 Pharmacists prescribing via clinics, undertaking polypharmacy reviews and signing prescriptions. Establishment of link workers across the Locality. Success in obtaining funding for 1 link worker to be based in a voluntary organisation to focus on social isolation. Trial of an open access mental health service. Engagement with care at home providers. Weekly meetings established from 18th March 2019. Significant improvement in securing support for those in hospital or at home.
Neighbourhood Planning Engagement at a neighbourhood level building on local capacity and assets. Wester Hailes, Oxgangs, Broomhouse, Gorgie/Dalry and Pentlands. Arranging Locality events to identify priorities.
