RULES AND RESPONSIBILITIES CONTRACT AZLE HORNET ATHLETICS RULES AND RESPONSIBILITIES CONTRACT Welcome to the Azle Junior High athletic program. We are excited to have another great group of athletes to prepare for high school. With a new school year comes a fresh start and renewed goals. The expectations are high for athletes for being campus leaders in the classroom as well as on the field. It is important to remember that athletics is not a PE class. It requires rigorous physical activity, as well as mental alertness and dedication to the program and teammates. Truth, honesty, loyalty, and commitment are required from athletes, parents, and coaching staff. Please read the following rules and responsibilities listed below. Forms – Each athlete must have a physical form, insurance waiver, emergency card, drug form, steroid form, acknowledgment of rules, and concussion helmet warning (football only) prior to participation in practice or games. (All online) Locker Room – Each athlete is required to bring a combination lock for his/her locker. This is required in order to maintain security for the athlete’s personal belongings. The locker room is for players and coaches only. It is also the athletes responsibility to keep the room clean. The locker room will be open after school for a brief time in order for your athlete to take home clothes for washing. Practices – Each athlete will be expected to be on the field/court at the time scheduled for practice to start. Arriving in time to get dressed in the proper practice attire is required. We ask that the athletes be picked up no later than 30 minutes past the completion of the afternoon practice sessions. Please be aware practice and game times are subject to change. Penalties will be enforced for missing practice/games for club teams. Workout Attire - The athletes need to wear the Azle Athletics t-shirt and black shorts of appropriate size and length. If your shirts are lost you may purchase additional shirts. No jewelry is to be worn during practice or games. Attendance – Each athlete should strive for perfect attendance at school and practice. An athlete has the responsibility to the team to attend every practice and game. We have practice plans that are based on every athlete attending. If for any reason you must miss, inform a member of the coaching staff prior to that practice or game. Illness or Injury - If you are well enough to come to school then you will be expected at practice. We will definitely take into consideration your illness or injury and modify the workout. Parents please be aware that notes from home will only be accepted for three days. On the third day a doctor’s note is mandatory. Note : If a practice is missed, it is at the coach’s discretion whether or not an athlete will have limited/any participation in the next game. Sportsmanship - Athletes will speak and act with respect towards all AJH personnel, fans, students, opposing teams, and game or meet officials. Participation - Participation in athletics is a privilege, not a right. Maintaining a minimum grade of 70 in each class is required for participation per UIL and State of Texas. Communication - It is important to have open lines of communication. Many times huge problems can be avoided by using the proper channels of communication. Please do not approach any coach after practice or games if you have a concern. Most of the time the problem can be resolved by contacting the coach first. If the problem cannot be solved, conferences are always welcome. They may be set up by email (bball@azleisd.net ) or (DAlsip@azleisd.net) Cell Phones - Cell phones must remain within school bag at all times while in the locker room and during the athletic period.

2019 - 2020 Coaching Staff Reminder Plan Reminders will be given for athletes who after counsel do not meet expectations. Reminder actions will be taken for, but not limited to, the following offenses. Action will not be taken without proof of infraction. 1. Athletes who receive SAC will not be allowed to participate in any games until SAC is completed 2. District policy will be followed for any student who is suspended or placed in AEP 3. Athletes that do not meet athletic expectations may be removed from athletics at the discretion of the school’s administration 4. Not suiting out properly 5. Stealing (possible dismissal from program) 6. Persistent behavioral issues (possible dismissal from program) Grading Policy Non-participation will result in a 5 point reduction per instance on the six weeks grade. 2019 - 2020 Coaching Staff Co- Coordinators – Barret Ball (boys) and Debra Alsip (girls) Girl Coaches: Courtney Palmer Neosha Beach Elizabeth Guardiola Boy Coaches: Brent Shaw Logan Enis Brad Morris Ricky Juarez Darrell Barron---Cross Country By signing below, I acknowledge and agree to Azle Junior High Athletics expectations and responsibility for this school year 2019-2020. _____________________________ Athlete Signature Date Parent Signature Date
