NMSEF / ISEF Required Forms ALL Project Proposals Junior or Senior Divisions

Student completes these fields and selections the Junior or Senior Division Category. This is a PDF that is “typable”. This is the cover sheet “Entry Form” and must be on the top of ALL project submissions to NMSEF. Submission deadline is 11/1/19 @ 5:00 pm to the North Museum. Categories may be changed up to 2/21/20. The bottom section is “for official use only” and is where the NMSEF SRC provides feed back and one of these three dispositions: Approved, Conditionally Approved, or Denied.

Form 1 is the Adult Sponsor form Form 1 is the Adult Sponsor form. This is a place for the adult sponsor to provide guidance to the student regarding the required forms for their specific project (see check boxes on this form). The Adult Sponsor is traditionally the student’s teacher. **TIP** Forms: Entry Form, (1), (1A), (1B), Research Plan, and in many cases (3) are required for all NMSEF project submissions. The other forms from ISEF are specific to the research conducted. The bottom section is where the teacher or Adult Sponsor completed their contact information. **TIP** If you have a lot of these forms to complete, write this once, save/print the file and use this as template for each student you have.

For 1A Student Checklist provides a wealth of data regarding the student and research project. The student completes these fields about their project #5 Please check “Yes” and the tentative start date is “11/12/19”. #7 Please leave this blank, this will be completed for the two projects sent to ISEF (NMSEF Champion and Grand Champion). If the project is completed at home and/or school, the address of these two locations is NOT needed.

The Research Plan A detailed procedure A Bibliography with at least 5 sources.

Box 1 on form 1B Approval Form provides NMSEF with an indication that the student, parent/guardian and supporting institution (i.e. the school), approves of this project proposal. Signatures in this box indicate the student and parent/guardian understand the rules, risks and ethical standards of ISEF and NMESF. 2a on form 1B. This signature indicates that the supporting institution of the student has reviewed and approved of the project proposal. This can be the signature of a “department chair”, “principal” or even another teacher at the school. It CANNOT be the Adult Sponsor of the student’s project. Box 2b is left blank for all projects. This final box, #3, is completed only for the two participants moving on to ISEF in the spring.

Some project may not require a Qualified Scientist to advise and review the research procedure. Thus Form 2 is NOT required for all projects. This form is completed by the qualified scientist that reviews the research proposal to ensure it is safe and appropriate. Their signature (in BLUE ink) indicated their approval. The need for an original “Blue” ink signature demonstrated the qualified scientist form is not a copy. The qualified scientist does not have to directly oversee the research. They may direct a “Designated Supervisor” to be in direct contact with the student during research. This is indicated at the bottom of the form.

The Risk Assessment Form (3) is a part of the proposal process to ensure a safe experience for the student researcher. While this form is NOT required for all projects, It is the recommendation of NMSEF that all project should have a risk assessment form. **TIP** Many of the “denied” projects would have been approved if any risks were delineated on this form. Examples of risk assessment data that can help with approval: description of power tool use, adult supervision while collecting sample (i.e. in a cave, stream, lake, etc.), non-household chemicals (with the SDS sheets), etc. The bottom section is completed by the designated supervisor (or qualified scientist when applicable).

Check-in Night & Project Setup March 11, 2020 from 3:30 pm – 7:00 pm @ Millersville University

The night of check-in and set up, copies of all Human Informed Consent forms required/collected for the project are to be shown at the check-in table. These forms will then be placed in a sealed envelope stored at the project. Sample

(2) and (3) turned in to NMSEF at Check-in Three (3) copies of the unofficial abstract should accompany the project to the Check-in and Setup Night. Affixed to the Project (2) and (3) turned in to NMSEF at Check-in The official Abstract Form will be completed for the two projects sent to ISEF (NMSEF Champion and Grand Champion).
