Core Component 1: Support New Member/Newcomer meetings Psychosocial Screening & Personal Support Plans Decision Assistance/Question List Counseling (Open to Options™) Group Support for people with cancer Group Support for Family, Friends, & Caregivers

Core Component 1: Support Cont… Groups/other activities for children/teens who have a family member with cancer or who have cancer themselves Cancer-related bereavement support for all ages Short-term cancer counseling for individuals and families (Optional) Awareness of & access to individual/group support online

Core Component 2 : Education Lectures/workshops about: cancer and its treatment psychosocial issues Skill-building programs Workplace, school & Faith-based education/support programs Awareness of & access to educational materials and programs online

www.MyLifeLine.org Interactive Blogging & Updates Helping Calendar Let family and friends know how you’re doing. We’ll email them when you post an update. Helping Calendar Coordinate volunteers to help with meals, rides to treatment, errands, child care, pet care, and more. Discussion Boards Connect with other patients and caregivers in an online forum to support each other with information, opinions, stories, and inspiration. Care Coordinators Make life easier by setting up family and friends as “Care Coordinators” to help manage your site and organize volunteers. Privacy Controls Your cancer experience is personal. Ensure the right people have access to comment on your site and volunteer.
