CIQN Updates Corporate Goal FY: Improve the Care of Children Within the Region (via health registries, community engagement) Created 2 new health registries Improved care for patients with Persistent Asthma by increasing controller medication prescription upon inpatient discharge(>2% above baseline) Increased Primary Care connection to IMPACT DC and then back to Primary Care or Pulmonary or Allergy Well Child/Well Adolescent registries currently in validation HEDIS dashboard available soon
