NEW INSULIN ORDER SETS (NON-ICU AREAS) ADULT HYPERTRIGLYCERIDEMIA ADULT NON-ICU HYPERGLYCEMIA BS GOAL 150-250 121-180 TITRATE INSULIN? No, Dextrose will be titrated based on BG Yes DEXTROSE REQUIRED? Always RECOMMENDED INSULIN BOLUS DOSE No bolus No bolus for BG equal to or less than 250 For BG greater than 250 bolus 5 units of regular insulin IV RECOMMENDED INITIAL INFUSION RATE 0.05-0.1 unit/kg/hour Based on BG BLOOD GLUCOSE (BG) MONITORING Every 1 hour If the BG is within the goal range (150 – 250 mg/dL) for 3 consecutive readings when performing every 1 hour checks, BG measurements may be performed every 2 hours. If the BG is within the goal range for 3 consecutive readings when performing every 2 hour checks, blood glucose measurements may be performed every 3 hours. Resume every hour checks if blood glucose falls above or below goal blood glucose range at any time. If the BG is within the goal range (121 – 180 mg/dL) for 3 consecutive readings when performing every 1 hour checks, BG measurements may be performed every 2 hours.
