What is Fit 4 Life? Fit 4 Life is for people who are: Type 2 Diabetics Pre-Diabetic (Hba1c 42-47) Aged 26-75 Research Project by NTU having gained NHS Ethic approval. We provide 12 months 1:1 support Specialist physical activity advice Signposting to existing quality assured local sport and exercise opportunities Sustainable physical activity. Working in partnership with: Healthier You NDPP Juggle Everyone Health And many more

Diabetes in Nottingham The 2015 Health Profile for Nottingham showed that 40,222 people in Nottingham and Nottinghamshire have Diabetes. Public Health England’s Health Profile indicated that recorded diabetes in Nottinghamshire was at 6.4% putting it significantly above the UK National average.

From Tai Chi in the local village hall or walking football on the park to joining the local cycling club More sociable More sustainable More affordable More accessible Run by the local community for the local community So far the Fit 4 Life gurus have identified and visited over 530 community based sessions within Nottinghamshire

All participants need GP consent to join the project The Referral Process All participants need GP consent to join the project GP consent can be provided via: System One/EMIS Email Fit 4 Life Consent slip The discussion about joining Fit 4 Life can happen through any patients health care professional i.e. Diabetes Nurse, Practise Nurse, GP or Dietician. All we need are patients contact details initially. Participants can sign up via the website however GP consent will still need to be sought.
