Three crew members died after entering this chain locker. The rusting of the chain reduced the oxygen levels to less than 10% This hole was cut in the forecastle to drain water. It created an oxygen deficient atmosphere that entered the store and killed two people This Halogen lamp was put in a chemical cargo hold after it had been cleaned. When the door was opened for inspection it exploded killing two people Examples of Accidents

Dangers !  Not enough oxygen  Poisonous gases (hydrocarbons)  People unaware of persons working in an enclosed space  No radio communication  Faulty equipment

Equipment BA System (Breathing apparatus) ELSA (Emergency Life Saving Appliance) ELSA Pressure Indicator Harness

• Risk assessment made to identify hazards. • Ÿ A list of work to be done should be made • Ÿ Opening and securing entry and exit • Ÿ Fire hazards identified • Ÿ The confined space is ventilated before entering. • Ÿ The space is checked to see how much oxygen there is ( 20% by volume) • Ÿ There is enough lighting • Ÿ A permit to work document is filled out and signed by the master • Ÿ Signs to tell people not to start equipment or machinery in that area • Ÿ One person to communicate with the person inside the space. Ÿ Ÿ Checks to be made
