2 What is the New DDD Assessment? The new DDD Assessment is a comprehensive, computerized assessment. It evaluates the specific, individual support needs of both children and adults with developmental disabilities who live in the community. It provides information to develop service plans when resources are available.

Why is a New DDD Assessment Needed? To assess the needs of each person with developmental disabilities To help Case Resource Managers plan for services and supports To give the Legislature and decision makers reliable and useful information

Why was it Developed? A legislative committee audited DDD and identified the need for an assessment process that would: Be consistent Be fair Be equitable Use an existing system Clarify policies and standardize practices

How Will it Help a Person with Developmental Disabilities? The DDD Assessment will evaluate: The supports needed to perform personal care tasks and to participate in the activities of daily living as fully as possible. The person’s support needs for employment, behavior interventions, medical help and caregiver needs.

6 How is this Different from the MPC Assessment? The MPC tool was originally developed for older people and was adapted for DDD to use. The New DDD Assessment evaluates for things that the MPC Assessment does not.

7 Differences (cont’d) The new DDD assessment is specifically designed to assess the unique needs of people with developmental disabilities. For example, it evaluates: Family caregiver needs, Behavioral support Protective supervision needs, Medical needs, and Employment support needs for working age people.

8 Who will Take Part in the New Assessment? The case manager will meet face-to-face with the individual and at least one other person who knows him/her well (most frequently this is a parent or guardian). Providers are also invited to participate.

Who Has Helped Create The New DDD Assessment? DDD has worked with many different groups to develop this new assessment: Individuals with developmental disabilities Parents Advocates Service Providers DDD Staff in the regions

13 Module 1: The Support Assessment Contains Supports Intensity Scale (SIS) Assessment for people age 16+ Contains the current Children’s Support Needs Assessment for children under age 16 Has additional questions about Exceptional Medical and Behavior Supports, Family Caregiver Needs The assessment ends here if the person does not receive any paid services.

17 What is the SIS? A standardized tool that measures the “habilitation” and training needs of people with developmental disabilities age 16 and older Focuses on what supports people need to be successful in community living

18 What does the SIS Measure? The SIS evaluates supports needed for a person in: 49 life activities, 8 protection & advocacy activities, 16 medical conditions, and 16 problem behaviors It measures type, frequency, and amount of time of supports a person needs to be successful

21 Module 2: Service Level Assessment Used to determine the service levels and number of hours for personal care (using the CARE system) Collects seizure and allergy information Identifies employment support levels for individuals receiving county services Asks additional questions, depending on which services the person is considered for.
