Early Childhood Whats new? All early childhood evaluations will be completed at the local school. This includes Child Find, Head Start and Early Intervention. Consent/assessment planning (domains) will be completed at the local school Schools will be held accountable for the completion of the evaluations Monthly information, updates and training around early childhood at case manager meetings Whats the same? All early childhood cases will be tracked from central office for completion. Families may go directly to their home school and request an evaluation for their child. Student must be enrolled as a non attending, and the evaluation must be completed. The general education teacher should be the Type 4 EC teacher and the special education teacher can be the LBS 1 or Early Childhood Special Education Teacher.
Case Manager Case Manager receives notification via SSM that a student has been added to the tracking OSES: OSES Enrolls child in CPS (if child is not already enrolled) creates Referral document in SSM uploads documents into SSM OSES: OSES Enrolls child in CPS (if child is not already enrolled) creates Referral document in SSM uploads documents into SSM Case Manager schedules assessment planning with the team and parent by creating a Notice of Conference and logging an event Central Office (OSES) receives referral for evaluation Parent signs consent for an evaluation. Team proceeds with assessments and eligibility. School makes 3 attempts to obtain consent. Each attempt must be documented in SSM under events. After 3 documented attempts within a 21 day period, the team will indicate Consent not Received and finalize the document. Consent for an evaluation was NOT received. Check their and SSM notifications sent from central office The SSM notifications will include the names of the students to be evaluated at your school by your clinical team Some students may be listed on your regular tracking report Other students will be found under Early Childhood Community Partnership *(Information on how to access these students is found under the HELP documents in SSM.) * The Early Childhood team is working with IMPACT/SSM to make the tracking report one comprehensive list. Currently, case managers must go through the EC Community Partnership list and locate the students assigned to their schools. Look for changes with this in the future! Case manager will:
Contact the parent to schedule the consent assessment planning meeting – Case managers are encouraged to contact the disability coordinators or referring agency to participate in the meeting and to facilitate the parents participation. – Document all communication and attempts to obtain consent as an Event in SSM » Once consent is received, the evaluation and/or eligibility meeting should be scheduled via a Notice of Conference (NOC) » The NOC is the tracking indicator used at central office to monitor progress of the evaluation. (Please note, the NOC must be created and finalized. Central office will follow up with the Principal and Chief Office if progress on the case has not been documented appropriately.) ….we do not obtain consent to evaluate? Create a NOC inviting the parent to review the Consent Assessment Planning document. Create at least 3 events indicating youve contacted the parent. If after 21 days, the parent has not returned consent, the team should address the Consent Assessment Planning Document by indicating Consent was Not Received and finalize the document. Remember to send the letter pre-drafted in SSM to the parent terminating the process. … What if………………..?
….we obtain consent but the parent does not present the child for the evaluation or the FIE? Send NOCs to the parent requesting a time for the child to be tested and/or hold the FIE meeting. After documenting at least 3 attempts, including one via certified mail, the team should move forward with the eligibility meeting. The team should review the existing information and note any data in the eligibility summary. If the team is NOT able to determine eligibility due to lack of information from testing, the child will be deemed Not Eligible. If the team HAS sufficient information to determine eligibility in the absence of the assessments, the student will be Eligible. Without the parents Consent for Initial Provision of Special Education Services, the process will be stopped. What if…………….? Instruction Support Services Unit
