Pre/Interconception Health Goal: Increase health care providers’ knowledge, resources and understanding of preconception health and improve pre/interconception health for consumers. Key Projects: Partnered on Interconception Care Project of CA (ICPC): Postpartum visit guidelines and patient handouts for women with pregnancy and delivery complications now available on From , funded 3 successful multi-year preconception and interconception demonstration projects. Best practice toolbox in development. Currently funding 2 multi-year grants to Sansum Diabetes Institute and to Sutter Medical Center, Sacramento - addressing preconception and interconception health for diabetics and teens.
