The issues Inconsistent service across the County Lack of data, therefore no monitoring or targets Relationships between officers are strained Reliance on agency staff and as a result high turnover of staff – poor quality reports There is a disparity and perceived inequality according to tenure There is no clarity of pathway for customers resulting in confusion about who does what and duplication i.e. low/moderate/critical/substantial needs Lack of information, guidance and publicity Lack of partnership procurement initiatives

Purpose of Proposal To enable individuals to manage daily activities in their own homes as independently as possible making use of techniques, equipment or adaptations

Objectives Access to appropriately trained and knowledgeable staff for assessment and advice. Complete range of options and choices identified early on in the process. Facilitate easy and prompt access to assistive equipment, minor adaptations, Telecare, safety and security measures. Consistent and equitable approach to assessment throughout the whole customer experience

What is required Local contact point and scripting Approved equitable procedure and resources for critical/substantial and low/moderate clients - Information and guidance (leaflets etc) - HIAs to access OT/Trusted Assessors - HIAs to access or have budget for equipment - Exchange of information - Project manager to co-ordinate the setting up - Speed in procurement Training in Personalisation - D & B’s up to speed

Role of In-house OT Holistic assessments for low/moderate and straight forward substantial adaptations. Support and guidance to trusted assessors / Handyperson. Advice and guidance for ‘self funded’ adaptations. Consideration of all housing options. Development of accessibility register within B/D Assessment of tenants who need to move to more suitable accommodation. Advising on suitable housing allocation and viewing of properties with disabled tenants. Advising on new build accessible properties.

Trusted Assessors Trusted Assessor Training course provides training for community staff in the assessment, use and fitting of basic daily living equipment. Could apply to handyperson, caseworkers, technical officers
