There is a saying - fail early – fail cheap; - fail late – fail expensive. This means that if a lacuna is not detected at an early stage, it becomes more difficult and, in most cases, more expensive to rectify it. These defects/lacunae often remain unnoticed at the early stages due to various of reasons like time pressure, insufficient knowledge / experience, cost consideration, etc. They are also sometimes caused by the change in the environment and/or application parameters as compared to its original design considerations / operating windows. Risk Management can be applied to a system and/or equipment and/or any identified business activity area. It is crucial to know what risks one is taking or is living with; and based on its operational criticality, cost, safety etc; proper technical & commercial view points ( findings & remedial measures ) need to be taken and suitable priority can be assigned to improve upon the same.
