Chronic Disease Prevention and Management Why is this focus area important? Chronic disease such as heart disease, stroke, cancer, diabetes and arthritis are among the most common and costly of health problems. Rates will rise over the decade as the average age of the population increases. These diseases can be prevented or mitigated in many ways, including healthy lifestyle choices.

Chronic Disease Prevention and Management Objectives: Promote sustainable chronic disease programs. Improve equitable access to chronic disease management. Reduce chronic disease health disparities.
