Across Kent we are working to make sure we Put people first by Person Centred Planning and person centred approaches Support local services Assist people to be part of their community Support outcomes for everyone Work in partnership Use resources well

Swanley Day Service  The day service building needs to be suitable for a greater range of people  12 people access the service but more might wish to in the future  The available space within the Junction building may reduce in the future  Person Centred Planning

A new Community based service means: Service Users will be able to continue to receive the same level of service but within a community based hub nearer the town centre Staff will work and be supported from a central hub These hubs will assist: Access to a wider range of activities Person-Centered approaches Opportunities to learn new skills Opportunities to move on into employment Greater choice

Who will we consult with? People who currently use Swanley Day Service Family and Carers Those who might want to use services in the future Staff and union representatives Other organisations, e.g. schools, colleges, Care Managers, DPG, Advocacy Services etc

How are we going to do this? Meetings for Service Users and Carers Meetings for Staff Individual meetings Information sharing events District Partnership Groups Further discussions with local community resources A twelve week consultation from: 7 th November 2013 to 31 st January 2014 Including:
