Evaluation  58% increase in referrals for older people  68% of those admitted to intermediate care following hospital discharge return home at the end of their placement.  22% reduction in care needs through the effects of reablement.  89 fewer LTC beds purchased now than in March  66% improvement in the Balance of Care.  Reduction of bed nights and ‘delayed discharges’ from hospital.
