Statements in this algorithm are derived from a critical appraisal of literature and a subsequent two round Delphi consensus process. Thus the levels of evidence and recommendations made follow the grading system used by SIGN and the last column in the tables which follow refer to the degree of consensus reached in the Delphi panel process where 86% was accepted as an acceptable level of agreement.
