Assessment Task P2 from last week Make 2 illustrated mind maps showing the socio economic and physical factors that can affect our health and wellbeing. Write a short rationale for each one, explaining their effects on an individual’s health needs and wellbeing. Include at least 4 examples on each mind map Use pages 58 – 59 to help you with your ideas. This task needs to be printed off now with your name at the top as I want to assess your work!

We can categorise the factors which influence our health and wellbeing needs into 4 categories. Socio Economic Socio Economic Physical Physical Lifestyle Lifestyle Health Health You now know all the factors that have an impact on health and wellbeing. Factors

Factors that Influence Well Being Task 2 (P2) Create a table that identifies the socio economic, Physical, health and Lifestyle factors that could have an influence on the well being of individuals. Include negative and positive outcomes for each one you list. Use P 58 – 62 to help you Positive/ Negatives Socio economic factors Health factors Physical factors Lifestyle factors

- What now? To achieve P3 you need to work in a pair to create a questionnaire that will help you to create a care plan for an individual. However you can already start this on your own if necessary. The questionnaire will be used to assess the factors that are influencing the health and wellbeing of a service user. It must cover all 4 areas of health and well being eg what you have just covered for P2.

Task P3 In a pair create a questionnaire with 10 to 12 questions that will enable you to assess the health and wellbeing of an individual. Next week these will be given back to you, filled in and you will create a care plan for the service user that has filled in your questionnaire. You will need to ask questions that help you to find out as much as you can about lifestyle, health, socio economic and physical issues influencing your service user, so choose your questions sensibly and carefully. I would suggest that you start with a text box that asks the service user to write a short personal statement so you can find out some information, as you only have 10 to 12 questions to ask.

Assignment 3: An Assessment of Health and well being. Task 1 (P3) Create a questionnaire that you will give to a service user to interpret their health and wellbeing. This should include questions based on socio economic factors such as environment, housing etc, health (including mental and physical health). Think about weight, fitness, illnesses or diagnosed conditions. You must also include well being: morale, self esteem, life style.

To end... Join another pair. Discuss the questions you have chosen to ask, are you going to be able to assess the physical, socio economic, health and lifestyle issues that are affecting your service user? Make any changes now and me your completed questionnaire. Next lesson you will receive them back filled in and then you will devise a healthcare plan for your patient.
