Where Are We Now?  From debate to conversation;  Critical mass of adopters and strivers;  Credible results confirming “it” works;  But the barriers remain persistent.

From Findings To Implementation What will it take to make resident-centered care the norm?

Seven Opportunities We Are Ready To Address  Further defining quality;  Changing the national conversation;  Helping leaders become change agents;  Engaging, and empowering staff early and systematically.

Seven Opportunities We Are Ready To Address (continued)  Involving consumers;  Adapting the physical environment;  Creating a unified will.

Opportunity #1: Further defining quality  The values and principles underlying culture change: Know each person; Each person can and does make a difference; Relationship is the fundamental building block of a transformed culture; Respond to spirit, as well as mind and body;

Opportunity #1: Further defining quality Continued Risk taking is a normal part of life; Put person before task; All elders are entitled to self-determination wherever they live; Community is the antidote to institutionalization; Do unto others as you would have them do unto you;

Opportunity #1: Further defining quality Continued Promote the growth and development of all Shape and use the potential of the environment in all its aspects: physical, organizational, psycho/social/spiritual; Practice self-examination, searching for new creativity and opportunities for doing better; Recognize that culture change and transformation are not destinations but a journey, always a work in progress.

Opportunity #2: Change the National Conversation… From what is wrong with nursing homes to thinking about long term care in terms of values.

Linking Quality and Outcomes Resident-centered Adaptations Improved quality of care and measurable Financial success Opportunity for practice, research and policy to view quality and efficiency as interconnected outcomes.

Opportunity #3: Address Identified Leadership Needs The importance of leaders as change agents in nursing homes that are both adopters and strivers.

Opportunity #4: Engage Staff At All Levels Earlier and Systematically  Expand the role of universities and colleges;  Integrate culture change into education and training programs;  Case Study: Nursing Home Administrators.

Opportunity #5: Engaging Consumers  Promising trends identified;  Actively create advocates for change.

Opportunity #6: Making the Nursing Home Feel Like Home  Update on the Creating Home Symposium;  Importance of activity on the state level;  The role of the Culture Change Coalitions and LANES.
