Example 1 - Barb calls in sick. She meets departmental call- in requirements. -She does not have FMLA nor has she indicated that the time she is taking is FMLA qualifying. Pay Code=SICK Comment=UNSCHEDULED

Example 2 -Jan is on an approved FMLA. -She calls in the day of her shift citing FMLA. However, she calls in late in violation of departmental policy. Pay Code= FMLA SICK Comment=AWOL- LATE CALL

Example 3 -Clarissa is injured at work. -She has filed a workers’ compensation claim. -She has opted not to use her accrued leave time. -She wants to take the time off as unpaid. Pay Code: WORKERS’ COMPENSATION UNPAID Comment: WORKERS’ COMPENSATION
