Online Notification Effective 5/23/11 Death Communication Form

Step 1: Place a Discharge Order The discharge order will have a disposition option of “Expired”

Step 2:Death Communication Form When you choose Expire, an order for the electronic Death Communication Form will automatically populate.

Step 3: Completing the Form The form mimics the paper form. Many fields are not required

Step 4: Finalizing/Printing Submit the order when you have entered information you currently have. Some information may be edited later, or written on the printed sheet. The form will print to the Nurses Station and to Admitting.
