Eastwood Booster Club

Eligibility Requirements  All students are currently eligible to participate  Students become ineligible if their GPA drops below a 2.0 at either midterm OR grading period.  Students who are absent, miss more than ½ the day, or assigned to ISS are ineligible to participate in the day’s activities (practice or game).  Attendance to practice and games is mandatory. If your child cannot participate for any reason, they need to communicate this 24 hours in advance to the coach. The coach will determine if the absence is excused. Players whose absence is unexcused will have consequences applied.

Requirements to Participate  Students must have an active physical on file dated on or after 4/1/14.  All signatures are required on the IHSAA physical forms including parents and players  Parents/students need to have read, understood, and signed the Concussion Acknowledgement Form and have turned it in.  All fees must be paid in full before first game.  Checks payable to Eastwood Middle School; your child’s name in the memo and sport

Fees/Costs to Participate  Various sports have various levels of fees associated with them; which are collected to pay for uniforms, equipment, and other expenses.  Individual fees must be paid in full to be eligible to participate in the first game*

Expectations for Participants  Don’t miss practices or games.  Inconsistent attendance makes it impossible for coaches to plan for effective and successful practices and competitions.  Try your hardest, always.  Getting better starts with your commitment to the sport.
