What will we do in this hour?  Understand the background  Test out a concept and idea’s  Get a shared understanding  Explore how the concept can work  Agree how to move the concept into realty

What is Community Brokerage/Advocacy?  Supporting people to take control of their lives and stay independent  Empowering communities to build capacity and knowledge  Enabling communities to support and protect the vulnerable  Non professional accessible and effective advice and guidance

Why do this?  Financial/Demographic  Health Inequalities  We are unique  Fits with NELSP and Neighbourhood Management  Delivers personalised care

What are the components? Neighbourhood Management level approach to;  Community owned advice & support  Collaboration between third sector & the community  Low level community based personalised care  Builds on existing examples- such as the Collaboratives and community groups

How do we deliver these outcomes?  Improved social capital and sense of community  Reduced dependency on statutory services  Improved take-up of services for disengaged or hard to reach groups  Better health and wellbeing of the population  Mobilise 5% of the adult population
