Coal Mine Fatal Accident OVERVIEW On July 11, 2011, a 26-year Supply man with 6 years experience, was fatally injured while operating a Brookeville 15-ton diesel locomotive pulling 2 loaded flatcars. The victim was located in the operators compartment with his head extending outside the compartment through a retractable door opening in the machine’s canopy. The victim received fatal injuries when his head struck a metal ventilation structure (overcast), while the locomotive was in motion.

BEST PRACTICES 1.At all times when a locomotive is in motion, any retractable opening on the operator’s compartment shall be kept in the closed position. At no time shall an operator extend any portion of their body outside the confines of the operator’s compartment. 2.All rail mounted equipment should be constructed to assure the operator’s field of vision is not obstructed. 3.When an abrupt change in the mine height occurs that creates a close clearance area, visible signs or warning devices shall be installed that would alert the equipment operator. 4.There should be a physical means to determine if the height of a load being transported into the mine will clear all close clearance areas; this shall be installed on the surface area at/or near the mine portal. 5.All operators of rail mounted equipment shall be trained in the safe operation of the equipment to be used and shall be familiar with all hazards associated with track haulage (to include all close clearance locations throughout the mine).
