Series 4- Team Meetings: you will learn: Purpose of PSP Team Meetings Participants In PSP Team Meetings Roles of PSP Team Members Referral process IFSP Team Meetings

What are PSP Team Meetings Bi monthly meetings which promote active participation of many professionals to receive coaching, support and to build on experiences with each other. The Primary Provider has the opportunity to learn new skills from other professionals and how to implement new ideas and skills during the home visit. Provides more occasions for sharing information, new ideas, feedback and self-reflection. Can help the Primary solve a current challenge. Provides time to discuss scheduling upcoming TCON, Periodic and Annual IFSP development.

Who Participates in PSP Team Meetings? PSP Team Supervisor Service Coordinators Community Providers Employed Therapist Family attends by invitation or request

Roles and Responsibilities of PSP Team Members PSP Team Supervisor develops the PSP Team Meeting agenda and facilitates the team meeting. Time Keeper ensures that team members stay on topic and keep the discussion going. Recorder takes minutes during the meeting and gives the typed minutes to Supervisor to disseminate to the rest of the team. Service Coordinator is responsible for ensuring that services are authorized accurately within CMS Kids including dates of service, provider names and payment sources are accurately reflected on page G of the IFSP.

Roles and Responsibilities continued: Primary provider is the PSP Team member working directly with the family/caregiver and child. They will assist the family/caretaker each week in maximizing natural learning opportunities, providing coaching and education on strategies to be incorporated into the child’s everyday routines. The primary attends bi-monthly team meetings to discuss challenges and obtain new coaching strategies to take back to the family. Secondary provider like the Primary, provides coaching and education directly to the family/caretaker, but on a less frequent basis such as once or twice a month for a shorter period of time. The secondary provider attends bi-monthly team meetings to discuss challenges to service delivery and obtain new coaching strategies to utilize with the family. Consult provides coaching and education to the Primary Provider. During PSP team meetings all providers have the opportunity to consult with each other to share strategies and gain additional coaching skills.

Referrals for Providers Referrals are made directly to PSP by the individual Service Coordinator. All unassigned children will be staffed during PSP team meeting for assignment. All referrals have to be accepted or declined within 3 business days. Referral forms are returned to Service Coordinator with initial date of service indicated on the form. Services must be implemented within 30 days of service authorization. Providers must submit POC within 30 days of service implementation.

Referrals for Providers Prior to discharge providers must discuss discharge with Service Coordinator to maintain accurate IFSP and ensure parents Procedural Safeguards are met. Prior to discharge provider must inform the family that they are discharging the child and when discharge will occur. To be followed up with a written prior notice regarding discharge to the family. Providers submits to Service Coordinator for each child they see Plan of Care, case notes and discharge summary.

Who Participates in IFSP Team Meetings? Parents Service Coordinator Primary Service Provider May Include Secondary Provider if assigned Consult as needed

IFSP Team Meeting’s Occur: When the parent requests an IFSP Team Meeting. When there are new concerns about the child's development. When there is a request for a change in service type, frequency, intensity and duration of services. At a minimum every six months the team meets to review the IFSP. Team meets annually to re-determine the child’s eligibility and develop new IFSP.
